Steps to Obtain a Work Permit Please bring the following items to our office at 1690 Huston Dr. 1. Copy of your birth certificate 2. Social Security card 3. Certificate of physical and completed form (click on link) Physical form 4. Statement of prospective employer completed form (click on link) Statement from Prospective Employer form In addition to all of the items above, your parent/guardian must accompany you to our office in order to complete the work permit.
